Applications and Questionnaires
Effective July 1, 2013, in accordance with our Tariff, the following requirements will affect all prospective customers.
Establishing service and supply of water shall be rendered only after:
1. Submitting a completed and signed Application
2. Submitting a completed and signed Water Use Questionnaire
3. Allow one business day for processing
4. Acceptance of the application by the water company
Residential Forms
To establish an account or update an account at an existing service address, please provide the new information on the appropriate form from below, sign it and check the “Update” box in the upper right-hand corner and send to us.
Forms may be submitted electronically to one of the following:

Forms can also be dropped off at our office or in the drop box at the front of our building located at 5410 189th ST E, Puyallup, WA 98375.
